Throughcare and Aftercare services

Throughcare and Aftercare Team

 

Throughcare and Aftercare Services for young people of Dundee are delivered through a Partnership agreement between Dundee City Council and Carolina House Trust. This has been in existence since 2003. This Partnership has recently been reviewed and a new structure has been put in place since January 2010.

 

The service has statutory responsibility to plan and prepare young people for leaving care and provide advice, guidance and assistance to young people who are no longer ‘looked after’. This is a statutory duty up to the age of 19, but authorities have a discretionary power to continue the support for a longer period in certain circumstances, eg if a young person is in further education and training.  

 

Carolina House Trust provides specific services to young people involved in Throughcare / Aftercare services. These are as follows.

 

Ø      A Reviewing Officer who independently chairs all Pathway Reviews for young people.

Ø      Two Support Workers who provide advice, guidance and support to young people through individual gap work programmes.

 

In the near future we will also provide a Supported Lodgings Service to young people. This service will be managed by a Social Worker who will recruit, assess and train Supported Lodging Providers.
